    How to Become A Substitute Teacher
    Forms & Documents for New Hires

    All candidates for teaching in the Buffalo City School District must complete the employment application process. This procedure includes the completion of all forms required by the School District, State and Federal Government Agencies. 

    The employment procedure is completed after you have passed your personal interview and have been invited via email to participate in a new Substitute Teacher orientation.

    Forms & Documents
    All of the documents listed below will have be printed and completely filled out.  These documents will be turned in during the new substitute teacher orientation for processing.

    To print the required forms click on the links below, complete them in their entirety (except for signatures that require a notary/commissioner of deeds) and bring them with you to the new Substitute Teacher orientation.

    Police Check Clearance
    In addition to the above forms, a police record check clearance must be obtained for first time substitutes, retirees and for anyone with a break in service of more than 30 days. This process requires the candidate to go to Buffalo Police Headquarters at 74 Franklin Street between the hours of 8:00 a.m. and 4:30 p.m. with identification and $10.00 processing fee (cash only, exact change), fill out the form requesting the police record check and submit the results to the Department of Human Resources during the new Substitute Teacher orientation/employment processing.

    All substitute teachers must comply with New York State Law requirements for a criminal check and fingerprinting.

    Required Documents
    In addition, Substitute Teacher candidates must submit the documents in box below to complete the employment process.


    The documents listed below must be received by the Department of Human Resources office before you can be cleared to substitute teach. In addition, Board policy and current New York State law require that we receive clearance from both your local police check and from the NYSED fingerprinting process before you begin to substitute teach.


    Official sealed transcripts from all higher education institutions which the candidate has attended or from which degrees have been received
    Two proofs of identification, one of which must be a photo ID. Acceptable proofs include a driver’s license, social security card, birth certificate, passport etc.

    Failure to complete any of the required parts of the application process may make the Substitute Teacher candidate ineligible for employment.
