    • Please scan the QR Code to the right to complete the Student Daily Health Screener. This form should be submitted electronically by a parent each morning that their child is attending in-person instruction.  This is a requirement for all children in attendance on their assigned days of in person instruction. This information is reported daily to NYSDOH.

    • Attention Grade 7 & 8 Families:

      If you have received a letter that your student will be assigned to attend the GRADE 7/8 summer school program, please click here or scan the QR code to fill out the registration form. The program will run from July 12th to August 6th and your child will scheduled based on the courses needed. Students will receive a NFTA bus pass on their first day of school if they live more        than .7 miles from the summer school location. If you have more than one child who has been assigned to the program, please complete a form for each child. Forms must be completed no later than June 7, 2021.

      Southside Elementary is, the largest elementary school in the Buffalo Public School District and offers over 1200 students in grades Pre-K through 8 a safe and quality education.
