Welcome to your internship experience with the
Buffalo Public Schools’ Career and Technical Education Department!The CTE Department offers three internship courses in which students may earn course credit. For a student to be enrolled in one of these courses, they must work with a certified work-based learning coordinator from the internship’s inception AND have passed CFM.

Internship Overview of Requirements
Checklist of tasks for student, work-based learning coordinator and employer for required tasks during internship.

1. Training Plan
This document needs to be completed first with employer, student and work-based learning coordinator outlining details of the internship experience. Internship may not begin until this document is completed and signed by all parties.

2. Memorandum of Agreement
Contract of internship requiring signatures of employer, student, parent, work-based learning coordinator, CTE program teacher & building administrator. All parties must sign before an internship can begin.

3. Emergency Medical Treatment Form
Needs to be filled out and signed by parent before experience begins.

4. Student Daily Log
Needs to be completed each day student attends internship and initialed by mentor.

5. Student Final Evaluation
Final grading of student at end of internship experience. Mentor should review with student and submit to work-based learning coordinator.

6. Student Evaluation of Placement
Student needs to complete at the end of internship experience and submit to work-based learning coordinator.

Accident/Injury Report
Required only if there is an accident or injury that occurs during internship.

Internship Summary Document
The work-based learning coordinator who oversees the internship completes this document and submits to CDOS Tracking System as proof of completion. This document can also be sent to guidance counselor(s) if an internship course credit can be issued.
