Manager All Schools Request Form

All School Access Request - Policies and Procedures

(For District Managers Only)

Note: All Schools access requests will be limited and are subject to cabinet level approval. Requests can only be submitted by upper management to cabinet for approval.

For Managers to get access to this application to submit All Schools access requests, they MUST follow instructions below:

(1) Manager must submit an email including the following District Personnel:
(a) Chief of Administration, Public Affairs, and Planning Dr. Will Keresztes (or designee)
(b) Chief Operating Officer, David Hills (or designee)
(i) CC: Director of Plant, Ken Sawyer (or designee)
(ii) CC: Security Systems Engineer, John Waterhouse (or designee)
(c) Chief of Security, Barbara A. Lark (or District designee
(d) Chief of Human Resources, Tami J. Hollie-Mcgee (or designee)
(e) Requestor's Cabinet level manager or supervisor (or designee)

(2) Manager provides within email justification of why you are seeking all schools access. This information should be detailed including all safety and security concerns.

(3) Manager provides detailed information including employee ID # and names of all staff in your department that you are requesting access for.

(4) Manager provides any detailed information including justification for any special days and times that they are seeking access for. (Note: 24/7 access is for emergency responders ONLY).

(5) Submit the email, and if your request is reasonable and justified you will get an email when you can access the application to submit via the form below.

(6) Once approved, submit the form online and it will go through an official documentation approval. You will be notified when the request has been completed. You will also be authorized to request personnel changes for these staff positions in the future using the form below.
