  9. BPS Membership Option:  Please note that all full-time instructional employees are required to join the retirement system.

  10. Teacher Retirement System Application:  This form will need to be printed out and have a notary signature.  Once complete, it can be sent to the Wages and Salary office in 719A City Hall.

  11. Advanced Credit Differential:  Please note that an official transcript must be submitted.  This must be sent electronically from the University to our HR team or sealed and mailed to the Wages and Salary office in 719A City Hall.  ALL graduate level courses that you are claiming MUST be listed on the ACD form. 

  12. Verification of Prior Service: This form will need to be sent to your previous employer(s).  Once complete, it would need to be sent to the Wages and Salary office in 719A City Hall or e-mailing them to our HR department.

  13. Tenure Verification Form: If you have previously been tenured in another NYS district, then this form will need to be sent to the previous district.  Once complete, it would need to be sent to the Human Resources Department in 720 City Hall or e-mailed to our HR team.

    Health Insurance and other eligible benefits - Please see below and review the benefits you are eligible for under for Contractual Bargaining Agreement (CBA). There are also required documents that need to be completed under the benefits sections. Benefit packages vary by union, if you move into a position that is under another union you MUST complete the benefits paperwork for the new union for are under.

    If You Change Positions

    If you transfer, promote, or move into a position represented by a different union, you MUST complete new benefits enrollment paperwork for the bargaining unit associated with your new position.

    Even if you are currently enrolled in benefits, a change in bargaining unit may affect:

    • Health insurance options

    • Dental and vision coverage

    • Life insurance benefits

    • Benefit contribution rates

    • Eligibility requirements

    Your benefits are an important part of your total compensation package. Please take the time to review and complete all required benefits paperwork.

    New Hire Enrollment: Employees have 30 days from their start date to select benefits.

    Life Qualifying Events (LQE): If you lose coverage or experience a life qualifying event (marriage, birth, change in bargaining unit), you have 30 days from the effective date of your LQE to add a spouse and/or dependent to your plan, enroll in your new plan.
