Application to replace lost or stolen Metro pass.
**Click below to open a form to request a replacement for a lost/stolen bus pass. A parent or guardian must fill out and sign form, once completed email or fax your request to metrotransportation@buffaloschools.org or fax 716-878-9744.

Once the application is received by the Transportation Department, the replacement pass will be printed and delivered to your child’s school within 5 working days.
